This group will offer a safe, welcoming and private setting for caregivers to share their caregiving experiences, connect with others going through the same journey and receive insight from an expert. There will also be group discussions and educational presentations about relevant caregiving topics.

For September the topic will be Successful Transitions for Individuals with Dementia. When transitioning from one environment/setting to another the fundamental principles that apply are: planning, communication, collaboration and person centered support.
